The forehead
lift is a remarkable procedure that corrects several problems at
once. Patients who have sagging eyebrows (most will also have
redundant upper eyelid skin) are the best candidates for a
forehead lift. Many of the upper facial wrinkles that are
temporarily improved with Botox injections are permanently
improved with the forehead lift. These include the frown lines
(between the eyebrows), the crow's feet wrinkles (in the temple
area) and the horizontal forehead wrinkles.
The primary
problem in aging of the upper face is descent of the forehead.
The frown muscles are brow depressors and act to pull the brows
down. Associated with a low eyebrow position are redundant upper
eyelid skin as well as heavy crow's feet wrinkles. The frontalis
(forehead muscle) counteracts the brow depressors and struggles
to keep the eyebrows in a higher position. Frequent contraction
of the frontalis causes deep horizontal forehead wrinkles.
Dr. Langdon uses
the elegant surgical technique developed by Dr. Niconar Isse.
Dr. Isse is a Los Angeles plastic surgeon who pioneered least
invasive endoscopic surgery for aesthetic applications. The
endoscope is a sophisticated miniature optical telescope that
gives the surgeon a "beneath the skin" view on a video
monitor. Small incisions are made in the scalp just above the
hairline to allow placement of the endoscope and of surgical
instruments. The brow depressor muscles are inactivated and the
forehead tissue is loosened to enable lifting. In the forehead,
the lifting occurs naturally through normal action of the
frontalis (forehead) muscle. Unlike other techniques for
forehead lifting, with Dr. Isse's method there is no need for
artificial fixation using screws or other devices. In the
temple, as in a face-lift, the fascia is pulled to a higher
level using sutures.
